Hair loss in men: early signs of baldness and treatments that actually work
How male pattern baldness starts, how to spot it early, what does and does not help, and how PRP and medical treatment fit in.

Many men notice it first in photos or under bathroom lights: the temples have moved back, the crown looks thin, or the hair does not cover the scalp the way it used to. Male hair loss is extremely common, often starting in the 20s, and the earlier it is treated, the more hair can be kept.
Male pattern baldness: what is happening
The most common cause is androgenetic alopecia, or male pattern baldness. It is inherited, from either parent’s side. In men with this tendency, a hormone called DHT (made from testosterone) gradually shrinks sensitive hair roots on the top of the head. Each new hair grows a little thinner and shorter, until some roots stop producing visible hair.
It follows a typical pattern:
- Receding at the temples, forming an "M" shape
- Thinning at the crown
- The two areas gradually joining
- The sides and back are usually spared, which is why hair transplants take hair from there
Early signs worth noticing
- The hairline at the temples moving back compared with old photos
- Hair at the front or crown looking finer and shorter than at the back
- The scalp showing through under bright light or when hair is wet
- More hair on the pillow or in the shower for several months
Shrinking hairs, rather than hair fall alone, are the key sign. Some men lose a lot of hair without obvious hair fall, because the hairs become fine before they disappear.
Other causes to rule out
Not every man with hair loss has pattern baldness. Other causes include:
- Shedding after illness, fever or stress, which comes all over the scalp a couple of months later
- Low iron, vitamin D, B12 or protein, especially with crash diets or heavy gym routines without enough food
- Thyroid problems
- Dandruff and scalp infections
- Alopecia areata: round, smooth bald patches on the scalp or beard
- Anabolic steroids and some supplements used for bodybuilding, which can speed up pattern hair loss
A proper scalp examination, and sometimes a few blood tests, sorts these out.
What actually works
Medical treatment
Treatments proven to slow male pattern hair loss and help regrowth include:
- Topical minoxidil, which prolongs the growing phase and thickens existing hairs
- Prescription tablets or solutions that reduce DHT, for suitable men after a discussion of benefits and possible side effects
These should be started with a doctor’s advice, used daily, and continued long-term, because the effect stops if treatment stops.
PRP therapy
Platelet-rich plasma from your own blood is applied to the thinning areas to support weak hair roots. It works best on hair that is thinning but still present, usually as 3 to 4 monthly sessions followed by maintenance. Many men combine PRP with medical treatment.
Other scalp treatments
Microneedling of the scalp, often combined with growth-factor serums or PRP, can boost the response to treatment.
Hair transplant
For areas that are already completely bald, a transplant moves hair roots from the back of the head. Even after a transplant, medical treatment is usually needed to protect the remaining native hair.
What does not work (or is unproven)
- Onion juice, special oils, and shampoos that claim to regrow hair
- Shaving your head to make hair grow back thicker
- Hair "vitamins" when you are not deficient
- Expensive treatments that promise full regrowth in weeks
Gentle oiling and good shampoo keep the scalp healthy, but they do not stop pattern baldness.
Habits that help
- Eat enough protein, especially if you work out.
- Avoid crash diets.
- Wash your scalp regularly, especially if you wear a helmet daily, and treat dandruff.
- Avoid tight caps and helmets that are worn damp all day.
- Do not use anabolic steroids.
- Manage stress and sleep. They do not cause pattern baldness, but they add to shedding.
How long until results?
- Hair fall slows in 6 to 8 weeks.
- Visible improvement in density takes 4 to 6 months.
- The best results show around 12 months.
The first goal is to stop further loss; any regrowth is a bonus. Men who start early, when thinning is mild, keep the most hair.
When to see a doctor
- Your hairline or crown is clearly changing
- Hair loss runs in your family and you want to act early
- Bald patches appear in the scalp or beard
- Hair loss came on suddenly or with other symptoms
The bottom line
Male pattern baldness is common, progressive and treatable, especially when caught early. Proven medical treatment, PRP for suitable cases, and healthy habits can slow it down and often thicken hair.
